MEDICAL REPORTS, CERTIFICATES & FORMS (Without an Examination) CHARGES from £15.00 to £50.00
Examples: Private Sick Note, Employment Certificate/Letter, Private Prescription, Private Medical Insurance, Shotgun Certificate, Holiday Insurance, Driving Licence, Seat Belt Exemption, Passport Form – Police/Teacher/Solicitor/Other Professional can sign your passport form
MEDICAL REPORTS WITH A FULL EXAMINATION & REPORT CHARGES from £80.00 to £200.00
Examples: HGV, Taxi, PSV, Council Reports, Employment Medicals, Complex Examination & Reports (Incl. Sports Certificate & Fitness, sky diving etc)
TRAVEL CERTIFICATES & VACCINATIONS CHARGES from £15.00 to £150.00 (Yellow Fever £50.00)
Fitness to Travel & Freedom from Infection Certificates, Private Prescription for Drugs (Solely for Travel), International Certificate of Vaccination & Replacement Certificates, Meningitis A+C, Yellow Fever Injection (Required 10 days before travel), Rabies a Course of 3 Injections (Payable at First Appointment). Hepatitis B for Travellers a Course of 3 Injections (Payable at 1st Injection)
Preventing illness is a priority at the practice. The Doctors and Nurses are happy to discuss Any concerns you may have. The practice offers specialised appointments that can be booked during routine appointments times for both health promotion and chronic disease management. Please ask at reception.
For long term problems that include Diabetes, Coronary Heart Disease, Cardiovascular problems, Asthma COPD
Book an appointment with one of the practice nurses who will advise on increasing awareness of risks, maintaining and improving diabetic control and monitoring long term problems.

The doctors provide contraceptive advice in normal surgery appointments.
Post Natal’s are carried out at the same time as the babies 6-8 week check and immunisations. This ensures the mothers health is assessed following delivery and that baby commences immunisation against infectious diseases
